You may fill these out in advance, but the completed forms must be submitted in person to the zoned school and cannot be submitted online. Certain forms may not be submitted without the required documentation needed for initial student registration.
Please bring the following required information when registering students:
- Copy of birth certificate
- Copy of social security card
- Shot record
- Proof of enrollment at previous school (withdrawal paper or final report card)
You must also bring Proof of Residency which must include personal property assessment and one of the following:
- utility bill
- rent receipt with current date
- lease agreement with current date
- dated contract for the purchase of a home
- dated contract for closing on construction of a new home
